I got it when I was around 30. But the only thing I experienced was an itchy blisterybspotnon the back of my neck, which was what I had asked the doctor about to get the diagnose with to begin with.

My FIL had it when he was like 65 or 70 and it was pure torture for him. 40 feels like it might be closer to my experience. I sure hope so, for your sake.
